**NHS Summary Care Record**
Patients should be receiving soon information on the new electronic record
called the Summary Care Record (SCR). This is a national system which will support patient care in urgent and emergency care settings. It will be accessible by any authorised health professional across the country. It holds very limited information, such as medication, reactions to medications and allergies.

**Your Oxfordshire Care Summary**
A new way of sharing health information locally. The Care Summary will only be available to support local NHS care in Oxfordshire and will have more detailed information than in the Summary Care Record (SCR).

Free NHS Health Checks
available at the Practice NOW !!
This check is part of a national scheme to help prevent the onset of health problems such as heart disease, type 2 diabetes, kidney disease or a stroke. Everyone between the ages of 40 and 74 who has not been diagnosed with the conditions mentioned above will be invited for a check once every five years. Even if you're feeling well, it's worth having your NHS Health Check now. We can then work with you to lower your chances of developing these health problems in the future. This is a rolling 5 year programme so not all eligible patients will have their check in the first year of the service running. An information leaflet explaining all about the new FREE NHS Health Checks is available to view and download under ' Clinics & Services'.
