  Friends & Family Test

With effect from 1 December 2014, the Practice is required to offer patients the opportunity to feedback their views on the care and service we have provided. This will be done by asking patients to complete a simple, 3 question form (either paper, in the surgery or online, via our website) every time they visit the surgery or have any form of contact with us.  To access the online form, please click on the Friends and Family tab to the right of this web page (under Have Your Say). Patients can complete a form as many times as they wish throughout the year and we will publish the results.  The Friends and Family Test is anonymous.

PLEASE TAKE PART IN THE

FLU, SHINGLES AND PNEUMOVAX VACCINATION

Vaccination to protect you against flu, shingles and pneumonia is still available at the Practice.

Flu vaccination is for patients aged 65 years and over, and for those in a clinical risk group.  Also, for those who are carers or who are pregnant.   Unfortunately, we are not able to offer flu vaccination for occupational reasons.

Children and young people in the clinical risk groups or who are aged 2, 3 and 4 years old (ie your child's birthday falls on or after 2 September 2009 or on or before 1 September 2012) are eligible for the nasal flu vaccine (Fluenz).

Pneumonia vaccination is available for those patients aged 65 years and over, or who have a chronic condition.

Shingles vaccination is available for those patients who, on the 1 September 2014, were aged 70, 78 and 79 years old.

Please ask at the Practice to check whether you are eligible for a flu, pneumonia or shingles vaccination and to book your appointment.
