Patients living within the area may register with our Practice and will be allocated a named doctor.
Children aged five and under should be registered at the same Medical Practice as their parent (s) / legal guardian (s).
It is very helpful to us to have your up to date mobile telephone number.
We would strongly recommend that you consent to receiving text messages from us. This is a very quick, useful method of communication that our team can use to forward information or test results to you, reply to your online queries, and we can use to send you reminders of upcoming appointments.

Acceptance on to the list is as at the discretion of the Partners and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
We have the right to refuse to register patients if they have a violent flag against their record.
Wallingford and Benson Residents Applications to Register with Goring and Woodcote Medical Practice
As of March 2022, Wallingford Medical Practice have reopened it’s list to new patients but Mill Stream Surgery have temporarily closed their list to new patients for 6 months.
In light of this, Goring and Woodcote Medical Practice are accepting applications to register Wallingford and Benson residents. Each application will be individually assessed by the partners to deem that it is safe and practical, as Wallingford and Benson are not within the normal boundaries of our practice. We will communicate this decision to you as quickly as possible.
If you are a Wallingford or Benson resident, please ensure you also complete and Out of Area application form which can be found under the registrations section of the Goring and Woodcote Medical practice website. This can be e-mailed to [email protected]
Pre-register online
Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please ensure you complete ‘Step 2 – Photo ID and Proof of Address’, as it may delay your registration with the practice.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online. Please ensure you complete this step as it may delay your registration with the practice.

Additional information
New Patient Health Review
We strongly recommend all new patients registering with the Practice attend for a health review. This helps us to get to know you and to be aware of any particular problems you may have. It can take some weeks before your medical record arrives from your previous doctor.
Organ Donation
From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.
Out of Area Registration
We now accept registrations from outside our Practice boundary. This does mean, however, that we will be unable to provide home visits to you. You would need to phone 111 instead to obtain a home visit.
This option may not be suitable for all patients. If you have any questions regarding this type of registration please contact the Practice. All requests for “out of area” registration will be considered by the Partners at their weekly meeting.

Usual Doctor
When you first register with the Practice, you will be assigned a “usual doctor”. This is the doctor who you will usually see when you visit the Practice for a consultation. This doctor will, therefore, get to know all about you and your medical history, which will enable us to provide you with continuity of care.
You may, of course, ask to see any other doctor in the Practice, at any time, or ask to change your usual doctor, if you wish.
Existing patients at the Practice will already have an assigned “usual doctor” but this may not be the doctor that they currently see. Please ask at the surgery if you want to know who your usual doctor is, and make sure that this is up to date.
