If you want to OPT OUT of GP Data sharing, NHS digital have not made it so easy:
Print and complete this form and hand in to the surgery. Copies will be available at reception for those who do not have access to a printer. They have also set a DEADLINE of 23rd June 2021 to do this.
The choice is an individual one, that only YOU can make, we cannot advise you on this.
The key thing is we want you to make an informed choice using the resources available.
